Presque Isle Downs & Casino Players Club Members who need a copy of their W2G statements or Win/Loss Statements for the prior year(s) can submit their requests online!

Please complete all fields on the form, and allow 2-3 weeks for processing and delivery. For any questions, concerns, or issues, please call us at 814-860-8999.
